READING DISCLAIMER & CONFIDENTIALITY AGREEMENT

By scheduling an appointment, by phone or in person, you are verifying that you have read and completely understand and accept the terms of this agreement.

In offering intuitive readings to the public, Annette Blackford, will not read for the following:

- Other people without  express written permission from that person and a return of this agreement and a reading request form by the individual to be read.

- Minors unless requested and accompanied by their legal guardian and/or parents.

-  Medical, psychological or psychiatric health care issues or concerns.  If you are in need of medical, psychological or psychiatric care, then you are advised to seek out care with a licensed medical professional for proper consultation and care.

The intuitive readings provided by Annette should never be used as a substitute for seeking professional care or assistance, therefore do not use the impressions given to you during a reading to replace any advice that has been given to you by a medical professional.  Anyone who is obtaining such professional medical help should continue to do so.  You also agree as with any information you receive from any source, you alone hold the power of decision-making and are totally and completely responsible for making decisions that affect your life and the outcome of your choices.  You agree that Annette makes ever effort to give you an accurate reading and that you understand that no intuitive is 100% accurate and you accept that some information received may not seem to apply at this time, but could apply to future events or may not apply at  all.  

Annette Blackford states that blogs, websites, radio shows, etc. disclaims all liability in regards to the intuitive reading and all information given to her clientele as a result of said reading.  By submitting to this agreement, you agree and verify that you:

- Have voluntarily decided to participate in this reading and have requested the reading of your own free will.

- You are 18 years of age.

- Understand the reading is purely for entertainment purposes only, and that readings once completed are not refundable.

- Understand that all information provided to you during a reading is considered confidential and shall not be disclosed except as required by law.

- Agree that Ms. Blackford can use portions of your feedback sent to her after the reading as a testimonial using your full name, city, state/country, without revealing specific information about the reading in order to protect your privacy.

- Agree to receive occasional future emails from Annette Blackford in the form of a newsletter, blog or notification of classes, readings and/or other information.
